Registration

Vaccine Requirements 

Parents, you are welcome to drop off your student’s vaccine record during office hours. We have collection boxes in front of school from 8:00am to 2:00pm. You are also welcome to scan or photograph the vaccine record and email to Margaret.mclean1@sweetwaterschools.org

*Students requiring prescription medications at school must present the following form filled out by the student’s physician. If the student can carry their own medication, please make sure the doctor marks that box.

Medication Authorization 

Documents required for registration:

  • Proof of Residency (see acceptable documents below)
  • Transfer grades and transcript from previous school
  • Current Immunization Record (including Tdap)
  • Special Education; current IEP (if applicable)
  • 504 Plan (if applicable)
  • GATE Letter (if applicable)

Proof of Residency (utility bill must be within 60 days of enrollment date)

Pay Stub, Utility Bill: Gas & Electric, Water, Cable TV, Internet, Trash or Sewer Verification of Social Services, Correspondence from Government Agency. Lease Agreement/Rental Contract with Letterhead of the complex, business card from apartment manager and a current rental receipt.

Joint residency requirements:

Parent/caregiver must provide all three:
1. California driver’s license OR CA Identification with current address
2. Current Bank statement OR verification of social services with current address
3. Current utility bill showing current address (SDGE, water, cable, internet, sewer, trash, pay stub, mortgage, correspondence from government agency)

Joint resident must provide both:
1. California driver’s license OR CA Identification with current address
2. Current utility bill showing current address (SDGE, water, cable, internet, sewer, trash, pay stub, mortgage, correspondence from government agency)
